Continuing Care
Continuing Care is designed to function as part of the individual’s overall transitional program back into the community. The Continuing Care component consists of weekly group meetings that are facilitated by specially trained alumni who have attended Continuing Care for at least one year.
The purpose of the group is for patients and family members/significant others to deal with the stress of daily living and to receive ongoing emotional support during the re-entry process. The Continuing Care groups have weekly topics ranging from Codependency to Relapse Triggers. It is recommended that the patient attend Continuing Care for at least four to six months or until they have successfully developed a support system that sustains sober living.
Continuing Care is an ongoing process that begins when the patient enters treatment. During discharge planning groups, patients, with the assistance of the Continuing Care Coordinator, complete their continuing recovery plan, examining life’s changes necessary to maintain sobriety and writing a program that will work for them. Included in this plan is 12-step involvement (daily meetings for the first three months recommended), possible referrals to Recovery Homes, individual therapists and/or family/marital therapists.
Progress notes and attendance records are kept while the patient is in Continuing Care. After completion of the Continuing Care component, telephone contact is maintained on a monthly basis for as long as four years.
Hoag’s Heroes Alumni Association
Patients are encouraged to participate in Hoag's Heroes Alumni Association, which is open to any patient, family member or significant other who has graduated from the Hoag Chemical Dependency Recovery Center program. Hoag’s Heroes meets on the first Wednesday of every month at 7 p.m. at the Chemical Dependency Recovery Center.
The Alumni Association plays an important role in introducing the patient to a social support network through sponsorship of such events as yearly picnics, an annual softball tournament and bi-annual surf retreats. Yearly functions such as Sober Cruises around Newport Bay, Deep Sea Fishing Trips and Golf Tournaments are held to raise funds that assist patients in need of a sober living environment following treatment.
