To receive a copy of your images on CD or film, please download the Film/Image Request Form.
Please print and complete this form and then fax it to 949/764-5861 for processing. Please note the instructions for retrieving your imaging studies are outlined on the Film/Image Request Form and vary depending on the study requested.
