Hoag’s comprehensive approach to women’s wellness

Your health history, diet, physical activity, and social and emotional health can affect your overall wellness. Incorporating new, healthy habits in each of these areas can enhance your quality of life today and in the years to come. Through healthy lifestyle modification many common diseases such as high blood pressure, diabetes and heart disease can be controlled or prevented before they occur. Positive lifestyle changes can also help control health issues such as fatigue, mood swings, insomnia and obesity.

The following is an outline of the important components of Hoag’s comprehensive approach to women’s wellness. Each of these services is provided by our multidisciplinary team of women’s health experts, who are committed to taking an integrative approach to your well-being; one that addresses all aspects of healthy living – physical, mental, emotional.

Detailed Health History Assessment

In order for us to gather important health and lifestyle information specific to your needs, a detailed health questionnaire will be sent to your home prior to your first visit. Our clinical team will use this vital information to help assess your current health status and evaluate your risk for disease.

Comprehensive Physical Evaluation and Health Risk Assessment

Our comprehensive physical evaluation begins with a thorough discussion about your health history with our women’s health nurse practitioner. This helps us to clarify your health concerns and wellness goals. You will receive a thorough head-to-toe physical examination as well. Based on your health history and physical, specific screening or diagnostic tests may be recommended to better evaluate your current or future risk for disease. Our nurse practitioner in collaboration with our physician who specializes in integrative medicine, will work together with you and each member of the Wellness Center team to develop an individualized Comprehensive Health Management Plan that will be provided to you and your personal physician at the conclusion of your visit.

Nutritional Consultation

One of the most important components of good health is proper nutrition. Understanding the direct impact of food on your metabolism, mood, hormone balance and medical conditions helps you make food selections that lead to optimal health. In partnership with you and the Center’s physician, our registered dietician will assess your individual needs and help you develop a sustainable, personalized nutritional plan that complements your lifestyle, food preferences and specific goals.

Fitness Evaluation

Individuals who are physically active are more likely to maintain a healthy body weight, reduce stress and most important, reduce their risk for major illnesses such as heart disease, diabetes and cancer. Our comprehensive fitness evaluation provides you with a thorough understanding of your current fitness level and how you can safely incorporate exercise into your daily goals for well-being. The fitness evaluation is conducted by our certified exercise physiologist, who will work one-on-one with you to develop a personalized fitness plan to meet your unique health concerns and wellness goals.

Lifestyle Counseling

In the busy day-to-day routine of juggling multiple priorities, stress can take a major toll on all aspects of a woman’s well-being. Hoag’s lifestyle counselor holds a PhD in psychology and has extensive experience in helping women to reduce stress levels in order to achieve a more balanced, healthier life. Lifestyle counseling is a unique and vital service offered as part of several of Hoag’s comprehensive wellness packages. This integrative approach takes into account the mind-body connection, and helps you to incorporate important stress reduction strategies as part of your overall wellness plan.

Laboratory and Diagnostic Testing

Our clinicians may recommend laboratory tests and diagnostic procedures that are important tools in assessing your current health status and risk for future diseases. An example is blood tests that investigate underlying health issues such as diabetes, underactive thyroid and other metabolic issues. We may also recommend specialized diagnostic testing such as bone density scans, mammograms, cardiac screening, etc.

After thoroughly reviewing your medical history and conducting a thorough physical examination, we will share with you what testing we recommend to develop a comprehensive analysis of your health status.

Some insurance providers require pre-approval for certain laboratory and diagnostic testing in order to provide reimbursement. We will help you determine what may be covered and whether or not prior approval is required. Please note: Any costs not covered by your medical plan will be charged to you, should you decide to move forward with the testing.

Comprehensive Health Management Plan

At the conclusion of each of our wellness evaluations, a Comprehensive Health Management Plan is provided. This detailed health portfolio contains an extensive summary of the results of the various assessments and testing you have received at the Center. And most important, it provides a customized action plan with personalized guidelines that serve as a roadmap to better health. Strategies for improving all aspects of well being – mind, body and spirit – are covered, as well as special educational references and tools for success to enable you to achieve your specific wellness goals and health needs.

An abbreviated version of this in-depth summary is also provided to your personal physician to ensure your health status and wellness goals are communicated to your primary health care team.
